This agreement releases Live One Life Coaching from all liability including injuries and/or effects that may occur on the premises and/or under advisement related to the nutrition, exercise, supplement, lifestyle habits and motivational coaching provided by Live One Life Coaching. By signing this agreement, I agree to forfeit the right to hold Live One Life Coaching liable, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.
I also acknowledge the risks involved in following a lifestyle program that includes nutrition, exercise, supplement, lifestyle habit and motivational changes. These changes include, but are not limited to possible change in physical, mental or emotional state and or wellbeing. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries either physically, mentally or emotionally while engaging in this program. I am responsible for informing Live One Life Coaching of any ailments, illnesses, medications, supplements and or concerns that may affect my program and will disclose all information freely without inquiring from Live One Life Coaching.
By signing below I relinquish and abandon all right to bring a suit against Live One Life Coaching for any reason. In return, I will receive nutrition, exercise, supplement, holistic, lifestyle habit and motivational coaching. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I ensure I ask for clarification when needed. I fully understand and agree without dispute to all of the above terms.
Purpose of Videos/Photos:Live One Life Coaching Promotion In consideration of the opportunity to participate in videos/photos produced by Live One Life Coaching, I agree that the video/photo may be used for Live One Life Coaching promotion, including but not limited to social media (i.e. Instagram, Facebook) throughout the world in perpetuity. I further agree that the videos/photos may be edited before use and that appearance in the video/photo confers on me no ownership rights. I release Live One Life Coaching, its employees, agents and assigns from all liability for any claims by me or any third party in connection with my participation in the video.
